The Connections Program

The Connections Program offers a distinctive model of therapy that bridges the gap between individual counseling and residential treatment. As a community-based integrative outpatient program, it emphasizes a comprehensive, team-oriented approach. Each client is supported by a licensed individual therapist, a licensed family therapist, a care coordinator, and one or more recreational coaches who assist with daily activities and promote engagement in everyday life.

Together—with input from the client and their family—our team of professionals develops a personalized therapy plan tailored to the client’s unique needs. While traditional counseling forms the foundation, our approach goes beyond the therapy room by integrating practical, real-life support into the client’s daily routine.

Depending on the client’s challenges and goals, coaches may assist with tasks such as completing homework, writing a resume, organizing their space, managing daily schedules, or participating in activities like hiking, bowling, or tennis to build confidence and engagement.

This individualized, hands-on approach is designed to help clients build the skills they need to function successfully within their environment and move toward what we call functional independence.
